Willow Physical Therapy Frequently Asked Questions:

Do I need a physician’s referral?
In the state of Alaska, a licensed physical therapist can provide “out-patient physical therapy” treatment without a physician’s referral. HOWEVER, it is in your best interest for you to call your insurance provider to confirm your insurance plan pays for physical therapy treatment without a referral.

Medicare and Medicaid recipients may also “refer” themselves to physical therapy; however, all physical therapy providers are required to have a treatment plan signed by your medical doctor and on file in your chart.  Therefore, we do require all Medicare and Medicaid recipients to bring a medical referral to the first visit. 

Is there anything I should bring?
You should bring your physician’s referral slip, if you have determined you are required to have one, as well as any pertinent reports you might have from your doctor/healthcare provider such as x-ray and/or MRI reports, or test results. If you wear a brace or prosthesis/orthosis, or use an assistive device, you should bring it as well. Lastly, please bring a list of current medications, amount taken, and reason(s) for taking this medication. 

What should I wear?
You should bring/wear comfortable clothing you would normally wear for exercising as much of your therapy will involve movement and/or exercise. For back related injuries/disorders we suggest you wear sweats. For knee related injuries/disorders please bring/wear shorts.

What about my regular doctor?
We do not replace your regular physician. You should continue to consult with him or her for all your medical needs. During your physical therapy, your therapist will communicate frequently with your physician and provide him or her with copies of your evaluation, progress reports, and discharge summary.

Cancelling an appointment?
Please call our office as soon as possible for any cancellation needs and to reschedule your appointment. We request 24-hour notice for cancellation.  907-456-5990

Do you bill by my insurance?
Willow Physical Therapy is a participating provider for Medicare, Medicaid, and Tricare; in addition our therapist are also “preferred providers” for Blue Cross/Federal Blue Cross, and as a courtesy, we do bill your insurance company.  We also bill for Worker's Compensation and Motor Vehicle Accident claims.

How do I pay my portion of the bill?
Our financial policy will be discussed with you on or before your first visit. Deductible and/or co-payment is required at your first visit and each visit following. A monthly statement will be mailed to you for your records. Visa and MasterCard are accepted.
