Dear Future Patient:

To a large extent, the speed and quality of your recovery depends on you! You are a partner in the treatment plan formed by your therapist and your physician, and with hard work and perseverance you will achieve the best possible outcome.

Before your first visit:

Contact your insurance and KNOW your responsibility for your physical therapy services. You may want to ask about: deductible, co-pay/coinsurance, if a doctor’s prescription is required for payment, and understand any other limitations regarding your physical therapy treatment. Doing this will prevent any unexpected payment requirements on your part.

For your first visit:

  1. Arrive 30 minutes before your schedule appointment time to fill out your patient intake forms. (if you print and fill out these forms at home and bring to your visit you may arrive 15 minutes early)
  2. Be sure to bring your insurance card and a photo ID.
  3. Bring a list of current medications, dosage, and reason for taking.
  4. Bring your prescription for physical therapy, if applicable.
  5. Be prepared to pay your co-pay/coinsurance, or deductible if applicable.

  3. Men's Health New Patient Forms
  4. TMJ/TMD New Patient Forms

What to expect at your first visit:

Your physical therapist will perform an evaluation of your need for physical therapy, this may include:

  • Further questions that pertain to your condition or injury
  • Review of any X-rays or MRI reports
  • Objective Measurements: such as range of motion, strength testing, gait analysis, balance, pain assessment, special tests, palpation. (PLEASE wear appropriate clothing that allows for easy movement)

A treatment plan will be formulated for your specific needs and options for treatment will be discussed. Treatment may include:

  • Manual Therapy
  • Therapeutic Exercises/Therapeutic Activities
  • Gait Training
  • Home Exercise Program
  • And various other physical therapy treatments specific to your condition or need

Your treatment schedule will be decided upon as well.
Please plan for your initial evaluation and treatment to take 1 hour.
