Do I need a physicians referral?
|
In the state of Alaska, a licensed physical therapist can treat
you without a physicians referral. It is up to you to call your
insurance provider to see if they will pay for your treatment
without one. Medicare and Medicaid recipients must have a
referral from a medical doctor.

Is there anything I should bring?
|
You should bring your physicians referral slip, if you have one,
and any reports from your doctor as well as any x-rays or test
results your doctor has given you. If you wear a brace or
prosthetic, or use an assistive device, you should bring it
as well.

What should I wear? |
You should bring or wear comfortable clothes. For back injuries
we suggest you wear sweats. For knee, please wear shorts. For
strengthening you can wear whatever workout clothing you would
normally wear to exercise in.

What if I have more questions?
|
Don't hesitate to ask your therapist any question about your
treatment. We want you to understand what is happening so that
you can learn to help yourself. If you are unsure of your
therapist's instructions or your home exercise program, please
call and discuss your concerns.

What about my regular doctor?
|
We do not replace your regular physician. You should continue to
consult with him or her for all your medical needs. During your
physical therapy, we will communicate frequently with your
physician and provide him or her with copies of your evaluation,
progress reports, and discharge summary.

If I have to cancel an appoint-ment? |
Simply call our office and reschedule your appointment with the
receptionist. Please call as soon as possible. We prefer 24-hour
notice, so that someone else can use your appointment time.

Do you bill my insurance?
|
Willow Physical Therapy is a participating provider in Medicare,
Medicaid, Blue Cross, Tricare and most other medical insurance plans. We
also accept worker's Compensation and auto accident insurance
claims.

How do I pay my portion of the bill?
|
As a courtesy we will bill your insurance company weekly. Our
financial policy will be discussed with you on or before your
first visit. We do ask for a co-payment on the first visit for
those whose insurance company does not pay 100%. We send you a
statement at the end of each month. We do accept Visa and
Mastercard.

Is there anything else I need to know?
|
Just this: to a large extent, the speed and quality of your
recovery depends on you! You are a partner in the treatment plan
formed by your therapist and your physician-and with hard work
and perseverance you will achieve the best possible outcome.
